Happiness meter in Confluence

We are looking for a simple way to retrieve frequent feedback from our Confluence users. Like the happiness meter at airports. A simple question "How are you today?" with a simple range of choices. 

Has anyone already implemented something like this? Maybe there's an app or a sample for an user macro?
