How to configure below fields?

I am trying to understand when I click on Service Restored on a ticket , how it asks me for these specific details? [Eg. Resolution Notes] . How to configure it?

Hi Chris, 

The view you get is called a screen, with (custom) fields created on it. 

In your example, "Requires testing" and "Service restored" are transitions on the workflow your particular issue is using (making this issues go from 1 status to another). 

A jira admin can make a new screen in the admin - screens section, and when editting the workflow, add the screen to the transition.
