However, the resulting layout does not have the epic field present.
A few weeks ago, it showed up again, but then it recently vanished. What am I missing?

It looks like you're using the New Issue View on a Classic Project.
The field should appear - can you check first if the issue being viewed is an Epic or a standard issue type?
Epic fields behave differently depending on the issue type:
If the correct field is your screen and it's missing from the correct issue type, let us know :)

Interesting, I didn't know to look for an additional Epic field, in this case Epic Link. Adding Epic Link to the layout resolved the issue.
As a side question, why did this appear for a few weeks and then go away again?

For a few weeks, Epic Name was visible as if it was Epic Link on normal story issues. Then it went away again. Like, stopped being displayed.

Epic Link is one of those fields which only displays on the view page once it has a value. Do you have an example where the story/bug has an assigned Epic Link, but does not display?
