Adaptivist TM4C says TM4J is not installed

Following setup is created at single host:

1. Conflucence server 6.15.4
2. Jira server 8.2.4
3. Apache reverse proxy (user connections only)

Application links are setup via localhost address. User access connectors are separate fro those are for application links. Links are all green.

TM4J is installed for Jira instance and created some test cases.

TM4C is installed for confluence instance.

When trying to insert any report macro in confluence page TM4C going error: 

 Test Management for Jira is not installed on Jira

Error message: 

You cannot add this macro because Test Management for Jira is not installed on Jira. Get online help about Test Management for Jira

How can I troubleshoot my setup and find an issue?
