Create sub task screen not showing tab

I've created a subtask screen that is configured to customize the 'Create New screen'. The new screen includes two field tabs as well as all of the custom fields. However, when I click on the Create Subtask Screen, it displays all of the fields in the same screen, but the field tab is missing. For the most part, all of the fields are displayed on a single long screen. Despite the fact that the Create New screen  has two tabs, I am unable to see them. Please assist with this matter.

Make sure you don't have any field selection in the "Configure Fields" drop-down at the top right of the form.

If you are not displaying ALL, then the tabs disappear.

Otherwise, for me, the create sub-task displays tab normally.
