Is the survey closed?  I get this error when I open it:

It looks like the webpage at https://docs.google.com/forms/d/e/1FAIpQLSfY2swUPg1pWIZoouw1NcOf_6irdulsiARnJAz0AOAlT6p8LA/viewform?usp=send_form might be having issues, or it may have moved permanently to a new web address.

It might just be me. I am getting the same error.  No worries though, I can skip taking the survey.
