Any suggestions on how to set up wiki page to gather information for team events, birthdays, anniv?

Would like a page to gather info much like a form.  Any suggestions on a direction?

Out of the box I don't think there is a way to capture information. You could use a 3rd party app like Confiform perhaps for that matter. Feel free to explore all other apps in the marketplace to find a more suitable solution.

Confluence pages have collaborative editing, so multiple people can edit the same page.

If you want it to look like a form, you could setup a table with what type of info you're gathering and have users fill it in. It doesn't have to get that fancy.

You could also use a Team Calendar to track events.
