How can I set up page approvals or sign offs in Confluence Cloud without a 3rd party app?

Hi @Okwuchi Justice Mac-Ajaelu and welcome to the Community.

The only way is to use the Page Status feature.

https://support.atlassian.com/confluence-cloud/docs/add-a-status-to-your-page-or-blog/

On its own, a page status is purely indicative.

But you can make it sort-of functional if you use page statuses in automation.
