Can I use the Product Requirements Blueprint / Template for other types of issue in JIRA. It would be really good if our team could use a template similar to this that created linked issues from a parent issue using a "created by" link? (Confluence Server 6.11.2)

I would recommend that you edit the template itself to include the headings you require. See Product Requirements Blueprint:
Customizing this blueprint
As no two products or projects are alike, you can customize the templates that are used by the Product Requirements blueprint - see Customizing the blueprint templates.
You might choose to edit the index page in a space to change the columns to be displayed by the Page Properties Report macro.
You might choose to edit the page template to:
- edit the headings or add additional headings
- change the instructional text that prompts users to enter information to suit your context
- add or remove rows within the Page Properties macro.

As for filtering Jira issues with a parent in a specific issue, you might need to combine that with another add-on such as ScriptRunner:

I think you may have mis-interpretated what I was trying to achieve. Our set up has tasks that are a "child" or "created by" of an issue that is not called an Epic or User Story. I wanted to use the template to create tasks that were automatically attached to the higher issue (that is not an Epic).

The only thing you'll be able to customize is text and macros in the template itself. If you need to change the Blueprint behavior, then you'll need to develop a new Blueprint in-house.
