Can't find "ConfiForms Form" macro

I'm trying to add a form within Confluence but don't seem to have the "ConfiForms Form" macro to start it off. I have the (definition) and a bunch of other ConfiForm macros though. What am I doing wrong here?

The macro you are looking for is called "ConfiForms Form (Definition)".
