Just in case someone else runs into this and is puzzled:
In your table for a Page Properties macro, you can't have both a header row and a header column and expect to display the data. The header row takes precedence over the header column and the header column/fields are not available for display in the Page Properties Report.
Cost me 20 minutes to figure that out, so I'm hoping to save someone 20 minutes of puzzlement sometime.
