Can you see a history of who has edited the permissions under "Space Tools"?

@Emily Lord, I don't think it's under Space Tools.  Take a look at the Audit Log under General Configuration -> Administration.  This will give you a system-wide audit of changes including space permissions.  You can even export to excel for easier searching/manipulation.
