I'm hoping someone may have an example close to this question that would be helpful. We have a form that has 5 questions. Once we have all users submit their answers to the form I'd like to have another form display the results along with checkboxes to allow users to select the responses that they see as most valuable. So the second form would show the results from the first form in a grid with a checkbox for each row which would allow users to select the rows they see as most valuable and submit the form. Looking at smart checkboxes I was able to do this with one field but I want the other 4 fields to be displayed as well. Any suggestions?

What you essentially are doing (in 1st step) is asking your users to fill in the form... And when want to read those answers and offer picking up the best
Smart checkboxes is the best option here
You can either have 5 forms to collect the answers (for each question) and have 5 smart checkbox group fields to read from those forms, or have just 1 form to collect the answers and then have 5 smart checkboxes to point at this form, but to different fields (separate field for each question)

(5 forms is also OK, and even better if you want to be able to deal with duplicate answers)
