Confluence On Demand - Any way to grab user input?

When using On Demand, is there anyway to build any type of form to obtain user input? I'd like to buid a simple form with custom prompts to get information from an integration partner.

If you are also using JIRA in OnDemand, use Issue Collector which is meant for this - https://confluence.atlassian.com/display/JIRA/Using+the+Issue+Collector

I don't think that functionality is available on any plugin for OnDemand Jira. You can search all the plugins available on this documentation: [Atlassian OnDemand and Add-ons (Plugins)|https://confluence.atlassian.com/pages/viewpage.action?pageId=315326753] and maybe find something useful.
