Create new child page using a form dialog

Hi Atlassian Community, 

Is there a way the I can create a new page using a form in a dialog?
sample fields in the form are Title, Status (dropdown), Members (Multi Select), and Description. 

Then on the parent page I can fetch all the data and display in a table.

I'm not sure if this workaround would work for you but you could :

1. Create a template
2. In the template, insert the macro Page properties and add the label "page-properties"
3. On the parent page, display the macro "create from Template
4. On the parent page, display the macro "Page properties report" and restrict to the label "page-properties" 

For sure in the page property macro you won't have a drop down or a multi select but you can add instructional text to guide the users.
