We do have lots of tutorials on our wiki site, and the one to start with is https://wiki.vertuna.com/display/FORMS/How+to+create+a+form+to+create+a+Jira+issue+from+Confluence

I suggest you to follow this tutorial step by step, as it shows and explains how to start with setting up ConfiForms forms to create Jira issues (in cloud)
