Creating a Template with similar characteristics of the blueprints

How to make that when creating a Template in Step 2: Fill in Template variables only show me those variables but not the rest of the content of the page/template.

 

For example, The pre-setup Decisions template. We get a pop-up window requesting the variables, then it shows us the page to fill out.
