Does Trello have an integration with Google Forms?

We'd like to be able to have a service ticket intake form via Google Forms that generates cards in Trello boards. Does that integration exist today? 

If not - what would you suggest for creating an intake request form that feeds into Trello?

You would need to create your own integration using Zapier or something else.  There is not an app that will do this for you.
