When using Forms plugin "Insert ‘Forms - Checkbox matrix’ Macro"

Getting below error

Error rendering macro 'checkbox-matrix'

null

No other options are given.

Please help on this

Can you provide more information on the macro that you are using? Is this a third party Plugin?
