Formatting a Confluence Form

I am creating an interactive user form which has several fields both Mandatory and non-Mandatory. Right now the formatting is the default one. Is there a simple way to format the contents except to use the CSS to handle the same. I found some issues using the CSS as it caused my entire form go out of shape. Please advise. Thanks in advance.

Hi Johnson,

Confluence doesn't support Forms out of the both, so you must be using an app. Which app are you using?
