I would like to create a form in Confluence that can be used to nominate employees for an award. The form needs to be available to everyone for completion, but the completed form should be visible only to a select group of people. Ideally, we would have an email notification going to the select group of people when a form is submitted. We would also like to be able to generate a summary of all submitted forms.
Has anyone done something similar?
