How do you create forms in Confluence?

I need to create forms inside confluence. How can I do that? Please advice.

You'll need an add-on, as Confluence is a web-based editor and not a form collector. 

Try Adaptavists "Forms for Confluence" and "Confiforms" as the well-established options on the marketplace.

Thank you for the suggestions. I tried confiforms but find it very clunky with macros based and not made for signatures. I will try Adaptavists forms for confluence.
