How to create a survey with branching?

I'd like to create a survey in Confluence with branches based on answers provided by the respondent. For ex:

Do you own a home? Yes/ No.  If the answer is Yes, it would display a question asking about the type of home. If they answered No, it would go to the next question.

Confluence doesn't have this functionality out of the box, but the Forms for Confluence add-on can do what you need:
