How to insert a new row in existing table using a form element?

I want a simple form to add

first name

last name 

address

The form should have 3 text fields and should add the record in a table in the page.

There is no way to do this (simple or otherwise) built into Confluence. You will need to search the marketplace for "form" and see if one would accomplish it.
