I need a form entry which is only visible when the heading is selected in a radio button above.

I need to submit an entry for either Options A or Options B, I have a dropdown field in my ConfiForm which has these two options. 

Once the user selects Option A in the dropdown, I want text input box A to appear, and the user selects Option B from the dropdown, I want text input B to appear. 

At the bottom of the page I would like both Option A and B in the data table.

And for the data view you can just have a TableView over your form
