Interactive Form to be sent by mail

Dear Community. 

I'm attempting to create a form to manage internal requests. This form will contain some basic information related to some job requests, a crucial field should a request ID which should be incremented for each new request. I'm using an HTML macro, but to send it by email, I should use PHP, and I'd like to learn how to use PHP code in Confluence from you. I read a previous article about installing APACHE and other tools, but this does not work because I need to create a form that will be used by multiple users, and I cannot ask them to install dedicated tools. Do you have any suggestions?
