I would like to use a variable for my DB name. In other config files (e.g. server.xml) you can use JVM variables (JAVA_OPTS) to parameterize settings. Is the same thing possible with confluence.cfg.xml? I've tried to use the usual ${variable.name} template pattern to no avail.

No. But you can set up the reference to the database as a jndi configuration which you can then configure in the server.xml .
