As we all know we capture several action items inside the postmortem confluence page. If we could convert those action items into jira tickets and link them under the main incident jira ticket then it would be really easy to follow up on action-item tasks. But how to automate this?

It's like the retrospective action items. You may list the action items on the Confluence page and insert (or create) Jira tickets for each item. You can make use of "Retrospective template" on your Confluence site that gives pre-built structure and format.
