In Confluence 4.1.6, when I'm viewing any list of pages such as in the page tree, any pages that have been "Recently Updated" have this blue icon next to them. Does anyone know the meaning of "Recently Updated"? Is it updated in the last x days, or updated since the last time I personally viewed that page, etc...

The blue dot means that the page has been edited since you last viewed it. Hope it clarify your doubts.

Thanks. How often does that information get refreshed? Is that a setting somewhere?
To test this, I clicked on a page with a "blue dot", then, about a half hour later, I went back to the list, and the blue dot was still there even though I had viewed the page, and the page had not changed during that interval.

By rights it should be every minutes as it included under Confluence indexing process. If the blue dot is still there after the page is refreshed and you have confirmed that there are no changes in between, there might be indexing issue.
Need to check you logs to confirm, but you can try rebuild indices from scratch to see if the blue dot still there:
