When using a pre-made how-to page template, the creator gets a box to fill in the title and tags.

 

Can this function be recreated for my own templated pages?

Welcome to Atlassian Community. It's nice to have you.

If you are referring to the pop-up box that you get while creating some pages from templates, then those are a function of Blueprints. Essentially, Blueprints are a collection of templates, but have some other actions that have been programmed, such as creating an index page and that pop-up box you are referring to. For more information, have a look at our Blueprints documentation. 

You can edit the individual templates within a Blueprint, but not the Blueprint itself. If you want to create your own, then it will require some programming language. You can learn more information about that here: Atlassian Developer > Confluence Blueprints.
