I would like to share following problem which are occurring to only some of our customer's users.
On Confluence 4.2.4, when you select "heading 1" from "paragragh" dropdown list in rich text editor, usually underline appears automatically.
However, in some of their user's case, underline does not appear. Anybody has similar experience? Please let me know.

It is most likely that the examples are from pages in spaces with different themes.
A theme can be applied to a space, which will then apply certain changes to the look and feel of all pages within the space. If the Documentation theme is active, then the h1 and h2 heading styles will have the underline, as shown. If the space is using the Default theme, then they will not. Your instance may have other 3rd-party or self-created themes available too.

It seems you are right, this problem only occurs in certain space.
I am asking the owner of that space to check which theme he's using.

Weird. To diagnose this issue, I would open the edit page in Chrome and use the style explorer to figure out what's different between the two cases. That's where I would start, anyway.

I used:
.wiki-content h1,.wiki-content h2,.panel h2,.panelMacro h2{padding-bottom:2px;border-bottom:none;}
and underlines disappeared.

Actually, this problem is happening on Internet Explorer and Chrome. Switching language to English does not resolve this problem.

What immediately stands out to me is the example without the underline is in a different language (based on menu items in the editor) to the one where there is an underline so there could be language specific stylesheets being applied?
I'd advise the same as Matthew and use Chrome or Firefox inspection tools to see what stylesheets and styles are being applied in each case and if switching languages consistently causes the headings to not be underlined.

Nice catch. I didn't see the non-Latin text on the drop downs. I bet that's it. There are several ways to make CSS language-specific, described here: http://www.w3.org/International/questions/qa-css-lang.en.php
