This would depend on the application you are using for your calls/messages, but for Skype you would create a link like this:
Replace 'call' with 'chat' if you want to open a chat message instead.
This isn't specific to Confluence, so you should speak with your provider on what the proper URL would be in order to initiate a call or chat.
