We have two cloud instances of Confluence. I access both of them with the same Office 365 Identity.
Are we charged two user licenses in this scenario, or just the one?

The "licences" are per system, not by user, so yes, you're using two different systems, so you need to pay for each.
Imagine it as if Confluence 1 has a licence for 50 users and Confluence 2 has a licence for 100. You get access to both via an account, each access counts towards the consumption.

licenses are on a per instance basis, so you would need to have a license for each instance, yes.
