Can I add a form to Confluence which employees can complete with accident/incident details?

I want to add a form to Confluence which will ask the employee a series of questions about their accident at work. I would like it have check boxes and fields for descriptions. Ideally the info would then be saved or sent to our email.

It is currently not possible to do with default confluence. you have to install a plugin for this purpose.
