If you create a page from an Atlassian default "How to" template, it gets a "kb-how-to-article" label and is then visible on an automatically created index page "How to articles".
We have a pre-existing structure / page for these articles. Is it possible to change the page that is used for the index page? I.e. can I configure Confluence to use "My custom how to page" rather than the "How to articles" page?

Actually, you could create multiple index pages by modifying labels. For example, an index page for pages with label kb-how-to-article2.
Then use the https://confluence.atlassian.com/doc/content-report-table-macro-317194676.html to create the listing.
You could also have multiple tables on a single page, that would allow you to categorize the articles.

If its not too much of a hassle im pretty sure you could just copy and paste the content your "Custom how to page" to the created index and it wont create a new one with every "How to" and should instead just use that one however much it is changed
Alternatively if you are planning on using multiple indexes you could just edit the blueprint for "How to articles"

Thanks - changing the content-report-macro seems to do the job and allows me to delete the auto-generated page.
