When we try to Add Space we get a Page Not Found error. The log file contains the following:

I just upgraded to 4.2.8 but we also had this issue in 4.2.3 (I believe that was our previous version).
Double check if your server base URL is set accordingly as how you access Confluence.

Turn off the default space logo in Confluence Admin -> Default Space Logo (the URL should behttps://example.atlassian.net/wiki/admin/configuredefaultspacelogo.action). This will allow spaces to be created.

Yeah, you're right. This is a bug that was introduced on the latest OnDemand upgrades and it's currently being tracked here - https://jira.atlassian.com/browse/CONF-29865. The report says that it is fixed, however please note that the fix was not rolled out yet.
