Collecting Anonymous Feedback

There have been many questions asked already about this topic that I have dug through without getting an answer.

Is there a way to gather truly anonymous feedback within Confluence? All the macros we've looked into aren't truly anonymous. For example: 

• Forms for Confluence shows the Sender in the results
• ConfiForms has a box you have to check to make it anonymous. I do not like that a page editor could just edit the form and uncheck that box. The form is not truly anonymous.
• Easy Forms (see picture below) also has a box you have to check to hide users. We do not like that a page editor could just uncheck that box. The form is not truly anonymous.

Are there any survey/feedback macros available that are truly anonymous and do not even have the option for checking a box to view users? Using Confluence 6.13.4. Thanks!

Likert Rating Survey provides truly anonymous feedback with Confluence. You can view the documentation here.
