Creating space stopped working after upgrade to Confluence 3.4.6

We have recently upgraded our Confluence to version 3.4.6, and since then the "Create Space" stopped working - the 'Create Space' page is displayed, but if I fill it and click OK, it just goes back to the same page, no space created.

I have tried to tun on logging setting to Diagnostic, but nothing is logged.

Any idea how to debug this issue?

It happens in both Google Chrome and Firefox.

In Firebug, everything looks good:

• AJAX GET on /ajax/spaceavailable.action?key=NAME after I fill the space key returns 200
• After clicking submit, POST /spaces/createspace.action returns 200 and the same HTML page content (the create space page)

Which browser is it? Have you tried in another browser?

Just a shot in the dark here, but have you checked your disk space? Can you add a page to an existing space?

Plenty of disk space, and no problems adding pages.
