Form Macro Builder application for Confluence

Hi 

I have just downloaded from Atlassian Marketplace the app Form macro builder for Confluence. 

My intent is to have the dropdown list solutions and forms solution when creating the content of a Confluence Page. 

I donnot know how to use these app (Form builder macro), I mean my Confluence Admin has installed it. I open a new Confluence page, insert a tittle, then I go to the + symbol where all the macros are. And here it is: the Form builder macro. 

I select it, and the macro's box appears a message: "Please, choose an app to display in configuration". I click on the pencil to edit, it opens a menu on the right with a display to shoose an app, I click on the arrow and nothing happens: no options. 

Could anyone help me?

It seems that you've successfully installed the macro but haven't created any form via the macro. Perhaps you can create a form first then add the macro to your page?
