How can I add a feedback widget like the one on Atlassian Answers?

I would like to add a similar widget to my Confluence. Does anyone know which plugin is used for this, if any?

Atlassian Answers is not a Confluence instance. This is the base product (http://www.osqa.net/).

You can buy new Atlassian product called Confluence Questions (plugin). this is very similat to Answers.

There's a screenshot in Consu's question, and he's asking about the widget in that screenshot. He's not asking about having a website like Answers.
