How create a wizard after creating the Blueprint on my Confluence Cloud

Hi  Atlassian devloppers ,

After creating a  Blueprint add-on based on node.js for my Confluence cloud. using the tutorial on the link bellow

i wanna  create a Dialog  Wizard that display a  "name " field  (name of the page that will be creating via BluePrint) just after the user click on the blueprint
