I am looking to add a feedback mechanism in Confluence much like (well, exactly like) the KB survey that used to be around. Does anything like this exist any more? As a writer it would be exceedingly helpful to get feedback on a page.

We at B1NARY are very close to releasing "Peacock" - an app that is made for collecting feedback from Confluence pages to improve the overall content quality.

I can see that you have had a few great options suggested already. The team a Kolekti also has an application that will serve your purpose here in "Forms for Confluence". You can create engaging forms and surveys using the intuitive builder without any complicated macros or coding knowledge needed and then see your results in several interactive graphs and charts.

Hope all is well. Have you found a solution that works for you? We have just developed a new product called Page Feedback for Confluence. It is designed to get feedback from the audience and provide a simple dashboard for content authors and moderators to take action based on the feedback received.
