I need to create a form

Hello all, I need to create a form for the questionnaire, I  installed this free app: Form macro builder for Confluence, the first part was easier and fast, I was able to create a form. But now I can't add to my confluence page, I can't find the way to add the form created to my page. 

Any ideas how to do it or what other free app can I use?

Hi @ivan.hansen welcome to the community!

You can try to click on the + icon in the toolbar and select the macro that is to be added to your Confluence page.
