I'm trying to create a robust intake form with multiple columns and a variety of field types (picklists, checkboxes, text, dates, etc.) that would be accessible by anyone with the link. Once submitted, it would create a Confluence Content Page with the columns, fields and responses displayed in the same format as the form, rather than a pre-defined issue type in Jira. I have used Raley IntakeForms for previous intake forms, but the limitations it has in formatting, and the fact that it has to be tied to pre-set fields in Jira, makes this not a viable option for this particular effort.
Any guidance on where to start would be appreciated.

I am a bit confused about the Confluence and Jira part of your question. An option to deal with forms in Jira may be ProForma, as it has a Confluence style editor to design your forms, you don't have to link all fields in your forms to Jira fields and you can do all kinds of stuff with the captured data afterwards.
At the same time, if you would like to collect information directly in Confluence, there's plenty of forms and survey apps available in the marketplace to support that scenario as well!

If you really want something more advanced have a look at ConfiForms
The app allows you to build custom forms (including you own layouts) and have lots of options for integrations. It can create Jira issues for you, Confluence pages based on your layout or on a Confluence template, send notifications and much much more
And of course - absolute flexibility on how you show collected data... table, card, calendar.. or may be wysiwyg!? - possible and supported!

Take a look at Confluence Templates. It's not quite a general-purpose form tool, but it does have some features for smart fields (users, dates, etc).

One approach I've used successfully is to create a template for filling in a form/page (as you are trying to do). And then create a page with a button that fires off the template for the user. This makes it a one-click process for the person filling out the form, and avoids having to train users to hunt through a huge list of templates for the right one.
