Nothing happens when i try to create a new document

I am trying to create new requirements document or new meeting notes docs, or even a file list however when i click create nothing happening.

when i open the console of the browser i see a lot of js errors, is it from the product or from my browser?

I can confirm that the Product Requirement page creation went through successfully on my end as per the following screen shots below:

The same goes to meeting notes as well. For now, I would recommend you to try reproducing the issue on the Incognito mode / other web browsers to confirm if this is a browser specific issue. Let me know how it goes.
