Adding an input field and submit button

Is there a way to add an input field and submit button to the Wiki? 

I have a target URL to which I would like to navigate the user to with the value entered in the text box. I might have to format the data entered in the text box. I used a HTML macro with some JavaScript. But it is by default turned off in the cloud version and my IS is not turning HTML macro back on due to security issues. Any suggestions?

I would suggest trying Run Self-Service Reports for Confluence

It did the trick for us
