Asking for page title and labels

I would like to create page template that works like the 'HOW TO" template in Confluence. When user try to use that template, it will first ask for "page title" and then ask for "labels". Does anyone know how to create such template? I searched for many hours but get nothing out.

In Confluence there is 2 types of Template, "basic" one that can be create from Confluence UI and more advanced one name Blueprint that require developement to be created. How To Template are Blueprints.

You should be able to develop blueprints and installed them on Confluence

If development is not an option, I know that there is some apps that allow you to create some from the UI but never tested them.
