Create section by a form in Confluence

Hey,
I'm completely new to Confluence so I need an advice if my idea is possible and if, how can I achieve it.
In a space I have a main page. On this main page I would like to have a button like "Create X". When I click on this button a pop up windows should appear with a customized form (textfields, checkboxes, multiple choice selection and so on). After I have filled in all the information I have, I click on "finish" in the pop up windows. This creates in a sub page of this main page a new section with all the information I have filled in the form before. I'm pretty sure it's possible… but it feels like there are several ways to solve it and so i'm confused what's the best way.
We are using Confluence Server in out company. Since there are a lot of restrictions it's not possible to install any other third-party macro/app.

You might be able to achieve a simple version of this by Creating a template which supports variables for test, multi-line text, and suggested selections from a list.

You'd then want to pair that with the Create from Template Macro on your main parent page.

But for checkboxes, and more advanced selections, etc. you'd really need an add-on. There's several, but here's a few:

Well, the closest thing to a checkbox is the Confluence task. The problem for using it in a form is, of course, that it can also be unchecked and checked in read mode and thus also accidentally.
