Custom form

I want to create a form which will have two fields:

1.Username

2.Department

The username field when on focus will auto complete with confluence users when I start typing the name.

The department field can be a dropdown.

When I submit the username and the department I want to save the result on a database whereby I can later retrieve the result and display on a page together with other user profile information.

Please assist on how I can achieve this.

Where do you want to create the form? Basically you can use Jira Rest Api or Confluence Rest Api for the task.
