I created a form using a template and then used 'create from template' on a page so that users can easily create a form to fill in.
I have a report via page properties that lists all the new forms. What I need is to be notified when new forms are created. I tried putting an '@ mention for my name' on the template but that does not work.
Is there a way to have the form email me or notify via workbox somehow?

Gail, having your mention on a template (assuming the user does not remove it before publishing) should send you a Workbox notification and an email.
I have tested this on a space on my local Confluence instance and works as expected.
If you insert the mention like for yourself, it should insert the mention when the user publishes the page based on your template.
Could you test with another user the following:
- User A creates a page and inserts a mention to you @Gail Welch
- in a page where you have access to
- The user publishes the page
- Check if you receive a notification
Also, could you share with us a screenshot of this template? Only if this has no sensitive information, of course. Community is an open space and anyone can see everything here.

thank you for your reply, here is the screen shot, there are 3 page properties macros as if only one then the data int eh other 2 tables does not get reported on.

I don't think I get the real issue behind. Are you able to insert a @mention in your template?
Could you clarify what means when you say that "...other 2 tables does not get reported on." ? Is this pointing to the fact that @mentions are not working as expected?
What happens if you type a @ character in your template while editing it? Are you able to type usernames and search for yourself?
Also, can you clarify if we are dealing with Confluence Cloud or Server? For Confluence Cloud, the URL would look like https://myinstance.atlassian.net.

using Confluence Server,
yes I can insert an @ mention into the template as per the screen shot above - I put @ and start typing my name, it comes up and I select it.
Re the page properties reports - I have 3 page properties macros in the template to display the information better visually to the user, the 3rd table 'Training Team Use' is where the @ mention is. The tables have to be in their own page properties macros so the the parent page 'page properties report' pulls all the data - which is does.
It is only the @ mention that is not working.
